Services to Adults with Severe and Persistent Mental Illness
The Community Support Services Division provides a comprehensive range of services for adults with severe and persistent mental illness. CSS utilizes a community support systems/case management model.

Case Management
Case Management services include assistance with benefits, housing, employment, accessing community resources, and ongoing education and support. Each consumer is assigned a case manager who is their main link to the CSS program and the community.

Crisis Residential
Crisis Residential care is offered at Breakthrough House a ten-bed facility. Breakthrough House serves as an alternative to hospitalizations and provides care 24 hours a day for up to 30 days.

Young Adult
The Young Adult team works with youth making the transition to independent living from institutional or behavior disordered programs.

Program for Assertive Community Treatment
Program for Assertive Community Treatment (PACT) is a multi-disciplinary team that is responsible for keeping very disabled consumers out of state hospitals.

Vocational Services
Vocational Services provide pre-vocational services, job development, and on site job coaching. Consumers may be assisted in pursuing educational goals as well.

Attendant Care
Attendant Care workers provide companionship and oversight for consumers who need additional support in living independently. Services are based on individual needs and may range from a few hours at a time in the person’s residence up to 24-hours a day in some group living settings.

The Resource Development Team
The Resource Development Team (RDT) provides two specialized services: an on site day program offering educational and recreational groups and intensive skill building in the consumer’s home, teaching the many tasks of daily living necessary to live independently.

Crisis Case Management Team
The Crisis Case Management Team (CCMT) is available 24 hours a day when a consumer in crisis requires immediate in-person assistance. The focus of the service is to help consumers to stabilize and successfully remain in the community, even during times of emotional crisis.
